PROSTATE ARTERY EMBOLIZATION (PAINLESS PROSTATE SURGERY)

A minimally invasive procedure to improve the urinary tract symptoms caused by benign prostatic hyperplasia (BPH) is called Prostate artery embolization. In prostate artery embolization, the procedure involves the use of microparticles injected through a small catheter. This blocks the blood flow of the branches of the prostatic arteries and all this is done to decrease the size of the prostate gland to reduce lower urinary tract symptoms. The risk of sexual side effects and urinary incontinence are lower in Prostate artery embolization than surgery. 

An interventional radiologist performs this procedure. To map the blood vessels feeding the prostate, an arteriogram is done during this procedure. This procedure called PAE is recommended for those patients who either prefer this instead of surgery or for those patients who are not eligible for the traditional surgery. 

 

PROCEDURE OF PAE

 

In the procedure of prostate artery embolization, the first step is to give local Anaesthesia. An Interventional Radiologist will then get access to the arterial system by piercing the radial artery or femoral artery. This is done with a hollow needle known as trocar and this is usually done by keeping the ultrasound guidance in mind. A guidewire is threaded through a needle and subsequently the trocar is removed. 

The inserted guidewire permits a 4 to 5 French sheath to get inserted into the artery. Then, through a catheter under digital subtraction angiography which outlines the anatomy of the blood vessels, a contrast material is injected through a catheter. This technique is used to locate the prostatic artery and also to advance a microcatheter to the ostium of the prostatic artery. The microparticles are inserted through the injection which prevents the blood flow to the prostate and results in a reduction in prostate size.
